About the role
- Together this team is responsible for providing and coordinating care for an intimate panel of patients in our neighborhoods.
Responsibilities
- Nurse Practitioners are integral members of our Care Teams as they provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan.
- Because we are not a fee-for-service provider and manage patients within a global capitation budget provided by Medicare Advantage plans, our only concern and motivation is to keep our seniors healthy.

- POSITION SUMMARY Our Care Teams include a Physician, Nurse Practitioner, Care Coordinator, Medical Assistant, and support from a Nurse Care Manager.
- As a key member of Sage Health care team, you will perform annual health assessments, complete post discharge visits and deliver cost-effective, quality care to assigned patients both in our centers and patients' homes.
