About the role

Responsibilities

  • Manage case assignments, draft service plans, and review case progress regularly.
  • Facilitate clients in achieving personal independence and assist in developing daily living skills.
  • Perform other duties as assigned.
  • This role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to ensure the delivery of effective and client-centered services.

Requirements

  • Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.

Nice to have

  • High school diploma or equivalent is required
  • a college degree in Human Services is preferred.
  • CPR certification is preferred.
  • Certification: CPR certification is preferred.
