About the role
- Help patients regain strength, restore mobility, and return to the activities that matter most in their daily lives.
- As a Home Health Physical Therapist, you'll provide one-on-one rehabilitation care in patients' homes, helping them recover from surgery, injury, or illness while improving function, reducing pain, and supporting greater independence.
- Working closely with an interdisciplinary care team, you'll educate and empower patients and caregivers while helping individuals make measurable progress toward their goals and a higher quality of life.

- You'll assess each patient's functional abilities and home environment, develop individualized treatment plans, and guide them through targeted interventions that improve mobility, balance, strength, and safety.

Requirements
- Master's or doctoral degree from an accredited Physical Therapy program
- Minimum 1 year of experience as a licensed Physical Therapist
Nice to have
- OASIS certification or willingness to obtain
- Strong clinical assessment and treatment planning abilities
- Excellent verbal and written communication skills
- Problem-solving and critical thinking capabilities
- Ability to work independently with minimal supervision
- Strong organizational skills and attention to detail
- Comfort evaluating and adapting to varied home environments

Compensation
- $93,000 - $116,000 USD
