- Shields Health Solutions is redefining patient care through health-system integrated specialty pharmacy.
- We're hiring an Ambulatory Clinical Pharmacy Specialist who will play a hands-on role in elevating patient care at a nationally recognized Academic Medical Center.
Responsibilities
- Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards.
- Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination.
- Support quality‑improvement and REMS initiatives.
- In this dynamic, patient‑centered position, you'll build meaningful relationships with patients and providers while delivering high‑quality medication management.
Requirements
- Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
- At least 3 years of ambulatory practice with disease state management
- Must be a licensed pharmacist in Oregon
- Completion of a PGY2 Ambulatory Care Residency
- Strong communication, organization, and tech skills
- Empathy, integrity, and a team-first mindset
Compensation
- An employee in this position who performs the job duties in Oregon can expect a salary rate between $140,000 and $170,000 (Hourly
- The actual salary will depend on experience, geographic location, and other factors permitted by law.
