About the role
- The Care Manager will be a vital part of our operations in Indiana.
- include performing orientations, providing an assessment of new patients to our program, and training our caregivers to ensure they provide the best service possible to their loved ones.
Requirements
- Training caregivers / patients as required per case
Nice to have
- 2+ years of experience working with frail or elderly disabled adults and caregivers
- Experience with data input, case review/case note entry
- Salesforce experience is a plus
- Hands-on experience in home care, preferred
- Working knowledge of ADL/ IADL assessments
- Valid driver's license with an insured vehicle for travel within the state
- Excellent verbal and written communication, organizational and time management skills
- Strong critical thinking and problem-solving skills
