About the role
- Prepares schedules to provide service to program clients.
- Coordinates all new referrals from external and internal sources.
- Answers the telephone and responds to general inquiries regarding the program.
Responsibilities
- Plans, implements, evaluates, and coordinates appropriate patient care.
- Collaborates with others to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures.
- Performs other duties as assigned.
- Coordinate and support the daily operations of assigned programs.
