About the role
- The Intake Coordinator serves as the primary point of contact for incoming hospice referrals and is responsible for coordinating the intake process from referral through admission.
Responsibilities
- Maintain accurate referral records and monitor the status of pending referrals.
- Maintain confidentiality and comply with HIPAA and all company policies.
- Collaborate with clinical leadership to ensure an efficient, patient-focused admissions experience.

Requirements
- Communicate with hospitals, physician offices, skilled nursing facilities, case managers, and other referral sources to obtain required documentation.
- Verify insurance eligibility and ensure all required documentation is complete prior to admission.
- This role requires excellent communication, organization, attention to detail, and the ability to provide exceptional customer service while maintaining compliance with hospice regulations and HIPAA standards.
- Qualifications High school diploma or GED required; Associate's or Bachelor's degree preferred.
