- We are seeking a dedicated Oncology Care Coordinator to serve as a consistent, non-clinical point of contact for patients and families throughout the cancer journey.
- This role is designed to reduce barriers to care, improve access to timely services, and provide practical and emotional support through close coordination with patients, providers, and interdisciplinary team members.
- As a small nimble team, this role will support vaccinations and intermittent phlebotomy in addition to care coordination.

- Serve as a primary point of contact for patients and families, helping them navigate appointments, treatment, and care coordination
- Help connect patients to accurate, up-to-date educational resources regarding their diagnosis and treatment, as directed by the clinical care team.
- Screens for potential barriers to care and collaborates with appropriate team members to address logistical, financial, cultural, and psychosocial needs

- Role Description
- Assist with scheduling and follow-up appointments across the oncology continuum (e.g., surgery, infusion, radiation)
- Support connection to support groups, counseling, social work, and other internal or community-based services
- Establish trusting relationships with patients and caregivers through regular outreach and touchpoints
- Participate in interdisciplinary meetings and escalate patient concerns or needs appropriately
- Ensure patients complete key milestones, such as diagnostics, consults, and treatment starts
- Document patient interactions and support activities in the EMR clearly and in a timely manner
- Work closely with RN Care Coordinators, Social Workers, and Clinical Teams to ensure seamless, wraparound care
- Support in phlebotomy and vaccinations, as needed
- Reporting: This role will report to the Practice Manager
