Responsibilities
- This position will support Cedar Falls (Deery Suites, Martin Suites), Grundy Center (Creekside), and Madrid (Madrid Home).
- Western Home Communities is seeking a Registered Nurse (RN) MDS Coordinator to lead the MDS/RAI process, care planning, and resident assessments in a dynamic long-term care environment.
- Manage MDS/RAI assessments and electronic submissions
- Support admissions, chart audits, and resident status changes
- Educate and collaborate with nursing staff, CNAs, PCP's, and families

Requirements
- (Required) Active Iowa RN license
- (Required) Graduate of an accredited RN program
- (Required) MDS or OASIS experience (long-term care experience strongly valued)
- Strong knowledge of clinical documentation, care planning, and regulatory compliance
- Ability to work independently in a fast-paced LTC setting
Nice to have
- (Preferred) Experience in PCC
