Manage patients assigned to your Care Team, ensuring the RN and Interdisciplinary team visit frequencies align with the patient's acuity level and clinical needs.
Develop and update care plans based on ongoing assessments and clinical needs as determined by the care team.
Collaborate with your care team to ensure patient's needs are being addressed as their clinical status changes
Manage patient medications, including implementing orders, maintaining current medication lists, assisting with medication refills, and assisting patients with setting up consistent medication administration methods.
Perform additional duties as assigned.

1+ year experience as a professional nurse within the previous three years.
Excellent oral and written communication skills required
Must have general computer skills and a working knowledge of MS Office and the internet.
Employees in patient, client, or customer-facing roles must be vaccinated against influenza.
Active, unrestricted New Jersey Registered Nurse (RN) license, with the ability and willingness to obtain licensure in other states.
Willing and able to work Monday through Friday 8:30am to 5pm Eastern Time, both in the field and remotely, with flexibility for potential evenings and weekends.

California and/or Oregon RN license, highly preferred.
Hospice or palliative care experience, highly preferred.
CHPN certification, or ability to obtain.
