Responsibilities

  • Manage patients assigned to your Care Team, ensuring the RN and Interdisciplinary team visit frequencies align with the patient's acuity level and clinical needs.
  • Develop and update care plans based on ongoing assessments and clinical needs as determined by the care team.
  • Collaborate with your care team to ensure patient's needs are being addressed as their clinical status changes
  • Manage patient medications, including implementing orders, maintaining current medication lists, assisting with medication refills, and assisting patients with setting up consistent medication administration methods.
  • Perform additional duties as assigned.

Requirements

  • 1+ year experience as a professional nurse within the previous three years.
  • Excellent oral and written communication skills required
  • Must have general computer skills and a working knowledge of MS Office and the internet.
  • Employees in patient, client, or customer-facing roles must be vaccinated against influenza.
  • Active, unrestricted New Jersey Registered Nurse (RN) license, with the ability and willingness to obtain licensure in other states.
  • Willing and able to work Monday through Friday 8:30am to 5pm Eastern Time, both in the field and remotely, with flexibility for potential evenings and weekends.

Nice to have

  • California and/or Oregon RN license, highly preferred.
  • Hospice or palliative care experience, highly preferred.
  • CHPN certification, or ability to obtain.
