About the role
- This vital role involves acting as an advocate for patients within an acute care setting until they are safely discharged to their homes.
- In addition to meeting with patients and their families, you will ensure a seamless transition of care throughout the discharge process, guiding them through their options and verifying eligibility for care.
Requirements
- Valid Driver's License and a reliable vehicle are required.
Nice to have
- 3+ years of experience in a clinical care coordinator, liaison, or case manager role preferred.
Skills
- Self-motivated with strong time management and communication skills.
