About the role
- Provide continuous treatment team care management services to children and youth with serious emotional disturbance or severe mental illness, their families, and other team members' consumers.
Responsibilities
- Respond to urgent/crisis consumer needs as appropriate to position with assistance of supervisor, crisis services, and clinic staff by developing plans and interventions
- Develop and maintain relationships within family unit and supports.
- Develop and maintain community relations through prompt follow through with contacts.
Skills
- Mentor new care managers on basic skills.
Compensation
- $42,000 (base pay) with additional incentive for language skills and behavioral health experience.
