About the role
- Interacts with all levels of medical staff to discuss relevant patient information as warranted.
Responsibilities
- Performs medication history taking for patients recently discharged from the hospital or as requested by the Clinical Pharmacist.
- Performs other related duties as assigned.

Requirements
- Good knowledge of retail or specialty pharmacy practice.
Nice to have
- Credentials/Licensure Required:
- Michigan Board of Pharmacy Technician registration/license
- Certified Pharmacy Technician (CPhT) with PTCB.
- Graduate of formal pharmacy technician training program
- BLS certification.
- Experience with patient outreach programs.
- Sales related experience.
Skills
- Ability to communicate in English (verbal and written).
- Ablity to work independently and collaboratively with staff.

- Records medication information in the electronic health record and updates existing information as needed to ensure accuracy of the patient home medication list.
- Documents patient cases in electronic medical record effectively.
- Provides education to reinforce patients' self-management of medications, including proper administration, medication adherence, storage and drug disposal.
