About the role

  • Interacts with all levels of medical staff to discuss relevant patient information as warranted.

Responsibilities

  • Performs medication history taking for patients recently discharged from the hospital or as requested by the Clinical Pharmacist.
  • Performs other related duties as assigned.

Requirements

  • Good knowledge of retail or specialty pharmacy practice.

Nice to have

  • Credentials/Licensure Required:
  • Michigan Board of Pharmacy Technician registration/license
  • Certified Pharmacy Technician (CPhT) with PTCB.
  • Graduate of formal pharmacy technician training program
  • BLS certification.
  • Experience with patient outreach programs.
  • Sales related experience.

Skills

  • Ability to communicate in English (verbal and written).
  • Ablity to work independently and collaboratively with staff.

  • Records medication information in the electronic health record and updates existing information as needed to ensure accuracy of the patient home medication list.
  • Documents patient cases in electronic medical record effectively.
  • Provides education to reinforce patients' self-management of medications, including proper administration, medication adherence, storage and drug disposal.
