About the role
- The Hospice Registered Nurse Case Manager participates as a member of the Interdisciplinary Team (IDT) to provide holistic care to patients and families.
- This role is responsible for comprehensive assessments, care coordination, and ongoing evaluation of patient and family needs.
- The Home Health RN Case Manager administers skilled nursing care to patients requiring intermittent professional services.
Responsibilities
- The RN ensures implementation and ongoing updates of the interdisciplinary plan of care to support comfort, dignity, and quality of life at end of life.
Compensation
- Schedule: Monday-Friday, approximately 8:30 AM - 5:00 PM (with participation in on-call rotation as required) Location: Roanoke and surrounding service areas Starting Salary: $75,000/year
