About the role

  • Location: Hybrid within 30min of New Lenox, IL Job Type: Full-Time Salary Range: $25-$30/hr.
  • Hours: 8:30am-5pm This position is not eligible for visa sponsorship.

Responsibilities

  • Document patient interactions and care plan updates in EHRs including Epic and Cerner.
  • Support LPNs, RNs and providers with intake coordination, scheduling, and follow-ups.
  • Partner with client operations teams to ensure seamless delivery of services.
  • Deliver training sessions on care protocols, systems, and operations to care management teams.
  • Support other operational functions as needed
  • Perform all job functions in compliance with applicable federal, state, local and company policies and procedures.
  • They will support the development of patient-centered, team-based care, as well as support primary care physicians (PCPs) and practices in managing their panel of patients using a population management informatics tool.
  • In addition, this role will support key operational functions such as member enrollment, staff training, and client operations, bridging the gap between clinical expertise and operational execution.

Requirements

  • Proven problem-solver, comfortable with high-volume outreach, and ability to multi-task.
  • Required Qualifications

  • Minimum of 3 years-experience in primary care setting or similar specialty.

Compensation

  • Salary Range: $25-$30/hr.

  • Conduct high-volume patient outreach via phone, text, and digital platforms.
  • Escalate clinical issues to LPNs/ RNs as appropriate.
  • Assist with patient onboarding and enrollment processes.
  • Identify process improvement that enhance patient and client satisfaction.
  • Adapt to evolving workflows, technology, and program demands.
  • Contributes to a positive experience for patients and families through courteous telephone and digital interactions, accurate and expeditious routing, as well as referral to appropriate clinical staff when necessary.
