About the role

Responsible for enhancing the patient experience by providing a seamless navigation process from diagnosis to survivorship.

Works with patients and families to answer questions, provide emotional support and solve issues concerning the cycle of care.
Performs and oversees transitional care management processes to recently discharged patients of HHC, providing the best possible care options for patients and their families.

Minimum required:
Experience - Two years nursing experience.
If completing virtual care activities that may include multi-state practice, an active compact/multistate license (eNLC) is required within 60 days of hire.
Education - Graduate of an accredited school of nursing
Certification -
If completing virtual care activities that may include multi-state practice, an active compact/multistate license (eNLC) is required within 60 days of hire. Employees in role prior to 11/09/2025 will have until their next renewal date to achieve compact status

Experience - 1 year case management

PHYSICAL REQUIREMENTS (Medium): 20-50 lbs

Lifting 50 lbs max
Carrying of objects up to 25 lbs
Occasional to frequent standing & walking, Occasional sitting, Close eye work (computers, typing, reading, writing), Physical demands may vary depending on assigned work area and work tasks.

Oversees the appropriate after care activities to include but not limited to Paramedicine, Home Health, DME, Rehab services, Educare, Pharmacy and transportation needs.
