Pathways Healthcare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team.
This role provides direct patient care in a home health setting while collaborating closely with RNs, physicians, patients, and families to ensure high-quality care and positive patient outcomes.

Monitor and document patient condition, vital signs, symptoms, and responses to treatment.
Maintain accurate and timely clinical documentation.
Support quality improvement initiatives and compliance standards.

Valid driver's license and reliable insured vehicle required.
