Responsibilities
- Issue Escalation: Recognize when clients are not progressing through treatment or require additional support, promptly escalating concerns to the appropriate team members to ensure timely intervention and continued case progression.
- Documentation: Maintain accurate and timely documentation in the case management system, recording treatment updates, appointment details, client concerns, and next steps while ensuring internal teams have the information needed to coordinate case progression.
- Organization & Follow-Through: Able to independently manage recurring client follow-ups, prioritize daily outreach, and consistently maintain communication schedules.
- Interpersonal Skills: Capable of building trust and rapport with clients, emphasizing active listening and empathy.

Requirements
- Experience Required: At least 2-3 years in a client-facing role with strong organizational and documentation skills.
