Responsibilities
- Deliver Patient-Centered Care
- Develop and implement individualized care plans in collaboration with physicians and the care team
- Drive Outcomes & Recovery
- Support safe transitions of care and contribute to discharge planning
- Identify additional patient needs and collaborate with interdisciplinary teams
- Collaborate & Communicate
- Partner closely with nurses, therapists, and physicians to ensure coordinated care
- Document patient care and progress accurately and timely within the EMR
- You build strong, trusting relationships with patients and families
