About the role

  • Provide transitional or supported employment services to facilitate client entry into competitive employment.
  • Attend trainings on recovery, psychiatric rehabilitation, trauma, supported employment practices, substance abuse and cultural competency.
  • Keep apprised of new research and developments in these areas through journals, web sites and literature.

Responsibilities

  • supports client/family in achieving rehabilitative goals; monitors medication compliance; provides
  • interventions and skill activities; develops skills training programs; interacts with clients and families;
  • maintaining appropriate behavior; develop community support; manages at-risk behavior and
  • performs crisis intervention.
  • client and/or family to needed services and supports, monitoring the provision of services and
  • supports, assessing the outcomes of services and supports, convening the Treatment Team
  • Assist individuals in developing and practicing the skills defined on their Person-Centered Plans.
  • Link individuals to community resources and peer supports to promote community integration.
