About the role

- The APP will play a key role in improving patient outcomes, ensuring continuity of care, and supporting CHP's mission of delivering accessible, quality healthcare.
Responsibilities
- Develop and implement individualized treatment plans
- Support discharge planning and continuity of care upon release or transfer
Nice to have
- Active and unrestricted PA license in the applicable state
- Graduate of an accredited PA program
- NCCPA certification required
- Current DEA license (or ability to obtain prior to start)
- Minimum 2-3 years of APP experience in primary care, urgent care, emergency medicine, or a related setting
- Strong clinical assessment and decision-making skills
- Ability to work independently within scope while collaborating with supervising physicians and interdisciplinary teams
- Excellent communication and interpersonal skills
Compensation
- Schedule: 12pm to 10pm on Thursdays, Fridays, Saturdays, and Sundays
