About the role

  • We want to be transparent about this as we continue discussions.
  • Hours: Saturday & Sunday, 8AM - 5PM EST or CST Provides overall triage assessment, case management and coordination via telephonic and telehealth care to patients.
  • Assures optimal coordination of patient care across the continuum for all disease management and triage patients as assigned.

Responsibilities

  • Documents all patient/staff interactions.
  • Maintains current knowledge of pharmacology, disease management and chronic disease processes.
  • Performs other duties as assigned by management.

Nice to have

  • Homecare Homebase (HCHB) experience is preferred for this position.

Compensation

  • $70,000.00 - $85,000.00
  • The base compensation range for this role is fixed, with a maximum cap of $85,000.00. We want to be transparent about this as we continue discussions.
