About the role
- Responsible for the overall discharge resource connection and aftercare planning needs and placement of the patient based on their individual needs and as directed by the treatment team.
- Participates in quality care monitoring for the department.
Responsibilities
- Implements appropriate activities and/or programs. Implements services to patients in accordance with their individualized service plan (ISP) and within the case manager's scope of practice.
- Collaborates with the patient and patient's family or significant others to implement an effective aftercare service plan, explaining the available options to the team, including the advantages and disadvantages of each option.
- Assures that discharge planning begins at admission. Obtains collateral input from providers and supports.
Requirements
- CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
- First aid may be required based on state or facility requirements.
Nice to have
- Previous experience in case management preferred.
