We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance

You will own the systems and workflows that power new market launches, ensuring newly eligible patients are accurately identified, routed, and supported through Outreach and into care.
This includes building for the teams responsible for enrolling families (our Outreach organization) and ensuring a seamless handoff into clinical workflows within Care Hub.
This role sits at the intersection of growth, operations, and care delivery, and is ideal for a product leader who enjoys translating complex, real-world workflows into scalable systems that drive both business and patient impact.
Own top-of-funnel patient journeys
Design and optimize the end-to-end experience from eligibility → outreach → enrollment → first clinical interaction
Partner with Outreach (call center) teams to improve conversion, efficiency, and experience
Drive product discovery and execution
Lead the full product lifecycle using a build-measure-learn approach
Define product strategy and deliver iterative improvements focused on measurable funnel performance
Partner cross-functionally

As a Senior Product Manager, you will shape how patients enter and engage with Imagine Pediatrics - from initial health plan eligibility through enrollment and early care experiences.
Build systems that support new health plan and market launches, ensuring accurate patient identification and onboarding
Deeply understand and document workflows across health plan data ingestion, eligibility logic, outreach operations, and care team handoffs
Become an expert in pediatric value-based care, Medicaid populations, and health plan partnerships
