- Care is delivered in collaboration with CHMG physicians, with a focus on optimizing health, independence, and quality of life for older adults.

Responsibilities
- Prescribe and manage medications in accordance with clinical guidelines and organizational standards
- Collaborate closely with CHMG physicians, nursing staff, Family Medicine Residents, and interdisciplinary team members
- Support and assist resident physicians in the care of geriatric patients
Requirements
- Master's degree in Advanced Practice Nursing or completion of an accredited Physician Assistant program
- Minimum of 2-3 years of clinical experience in geriatrics or primary care for older adults
Nice to have
- National certification in specialty area preferred
- Experience with home visits and/or nursing facility care strongly preferred

- Maintain accurate, timely, and thorough documentation in the electronic medical record
