About the role

- As a Licensed Practical/Vocational Nurse, you will provide direct, compassionate nursing care to hospice patients under the supervision of a Registered Nurse.
- You play a critical role in managing comfort, supporting families, and ensuring every encounter reflects our values-trustworthy, capable, compassionate, proactive, and called.
Responsibilities
- Implement current nursing practice following a comprehensive assessment and the established plan of care.
- Deliver appropriate pain and symptom management; monitor patient response to treatments and medications.
- Document all care accurately and timely in alignment with the plan of care.
- Perform other duties as assigned by the Registered Nurse.
Requirements
- Graduate of an accredited practical nursing or vocational nursing program.
- home health, community-based, or medical-surgical experience ideal.
- Current state LPN/LVN licensure.
- Current CPR certification.
- Demonstrated adherence to professional nursing standards and practices.
- Licensed driver with a reliable, insured automobile meeting organizational requirements.
Nice to have
- Minimum of three years of nursing experience preferred
