Responsibilities

  • Patient Outreach & Engagement: Manage a structured outbound referral queue, contacting prospective program participants with consistency, warmth, and professionalism.
  • Complete and document standardized clinical intake questionnaires in real time.
  • Navigating Hesitation: Build rapid rapport with older adults who may be skeptical, confused, or reluctant, using empathy and clear communication to overcome barriers to care.
