What Makes This Role Unique Direct Mentorship You will work closely with a fellowship-trained geriatrician who enjoys teaching and case discussion.
Expect active collaboration, real-time feedback, and meaningful clinical development.
Purpose-Driven Work: You will play a vital role in caring for medically complex older adults during their most vulnerable health transitions.

Perform preventative screenings and lead important discussions regarding long-term goals of care.
Review and update patient medication lists to ensure accuracy, and order appropriate laboratory and diagnostic procedures Maintain and review patient records, charts, and other pertinent information accurately and efficiently.
Maintain a high level of independent clinical decision-making in a fast-paced environment.
Ensure patients receive care across the entire continuum and provide secondary support within various post-acute settings such as AL/IL and SNFs.
We invest in our providers' development and support their journey toward excellence.
