About the role

  • Locations: Will include travel to other locations 2 to 3 days per week (Possible locations may include Huntersville, University, south Charlotte).
  • Assess patient learning needs, goals, and barriers to care, and tailor educational interventions accordingly.
  • Provides formal instruction on diabetes management to the patient (inpatient and/or outpatient).

Responsibilities

  • The educator will maintain knowledge of diabetes management technology and manufacturer protocols.
  • Document and charge for services provided consistent with CMS and payor requirements and guidelines for DSMT and MNT services.
  • Employees must follow any other job-related instructions and perform any other job-related duties requested by their leaders.

Requirements

  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
  • Licensure, Registration, and/or Certification Required: Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian, in the State of applicable state.

Nice to have

  • Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian, in the State of applicable state.
  • If not certified, you must obtain certification within two years preferred but not to exceed 3 years.
  • Degree in Nursing, Dietetics, or other related disciplines that meet the certification eligibility requirements for CDCES.
  • If nursing degree, BSN preferred.
  • Master's degree in Dietetics required if Dietician license obtained after 01/01/2024.
