About the role
- This position involves contacting patients with abnormal results from screening events and providing them with next steps and guidelines.
- The role is structured around scheduled events occurring on a weekend shift, with a minimum of two (2) Sundays per month.
- MAJOR JOB DUTIES: Contact patients with abnormal screening results to discuss findings and provide guidance on next steps.
Responsibilities
- We provide program management software and program design services that let employers, schools and communities administer testing to prevent the spread of communicable diseases.
- Develop and communicate personalized care plans, including necessary follow-up actions and preventive measures.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
- Maintain accurate and timely documentation of patient interactions and care plans.
Requirements
- Minimum of 2 years of clinical experience, preferably in a preventative care setting.
