The Population Health Pharmacy Technician I for Stellus Rx will help our communities thrive as a key member of the Adhere Team.

Support medication reconciliation processes for patients within the designated population.

Proficiency in using pharmacy software and computer systems for data entry and analysis.
Ability to work collaboratively in a multidisciplinary team environment.
Ability to speak fluent Vietnamese.

You will work closely with Stellus Rx leaders and across the organization, as we work collaboratively to unlock the health of millions of Americans by turning "use as prescribed" into a guarantee, not a direction.
They collaborate with healthcare professionals, patients, and stakeholders to ensure effective medication management and adherence.
Participate in population health programs aimed at promoting medication adherence and preventive care.
Collaborate with pharmacists and healthcare providers to optimize medication regimens.
Work closely with healthcare teams to ensure seamless coordination of pharmaceutical services.
Communicate with patients, caregivers, and healthcare professionals to address medication-related concerns.
Support initiatives focused on improving health outcomes and reducing medication-related disparities.
