  • This role includes overseeing behavioral health assessments, providing telehealth support, and implementing behavioral health screening tools to optimize patient care outcomes.
  • Collaborate with primary care providers, psychiatrists, and other healthcare team members to integrate behavioral health services into patient care.
  • Administer, score, and interpret validated behavioral health screening tools (e.g., PHQ-9, GAD-7) to identify patient needs.
  • Act as a patient advocate to ensure timely access to appropriate behavioral health services.
  • Educate patients and families about behavioral health conditions coping strategies, treatment plans, and available resources.
  • Monitor and document patient progress, modifying care plans as needed Assist in the development and implementation of behavioral health programs to meet patient and community needs.
