About the role

  • Coordinate: Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.

Responsibilities

  • Our clinical leadership and intake teams are available for real-time field support when you need orders or coordination.
  • Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
  • Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
  • Empower patients and families to manage their conditions safely at home.
  • Maintain accurate, timely clinical notes on company-issued iPads.
  • Manage Your Caseload: Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
  • Skilled Care: Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
  • Educate: Empower patients and families to manage their conditions safely at home.
  • Document: Maintain accurate, timely clinical notes on company-issued iPads.

Requirements

  • LICENSE: Current, active California RN License is REQUIRED to apply.
  • Skills: Strong clinical judgment, organization, and the ability to work independently.

Compensation

  • $135,000 - $156,000 per year
