About the role
- Coordinate: Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.
Responsibilities
- Our clinical leadership and intake teams are available for real-time field support when you need orders or coordination.
- Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
- Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
- Empower patients and families to manage their conditions safely at home.
- Maintain accurate, timely clinical notes on company-issued iPads.
- Manage Your Caseload: Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
- Skilled Care: Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
- Educate: Empower patients and families to manage their conditions safely at home.
- Document: Maintain accurate, timely clinical notes on company-issued iPads.
Requirements
- LICENSE: Current, active California RN License is REQUIRED to apply.
- Skills: Strong clinical judgment, organization, and the ability to work independently.

Compensation
- $135,000 - $156,000 per year
