- Position Details: Main Office Location: Londonderry, NH Service Area: Exeter, Epping, Raymond, Danville, Kingston, Hampton, Seabrook, Portsmouth, Stratham, Durham, New Market, Newton, Fremont, Brentwood areas.
Responsibilities
- Develop and revise individualized plans of care based on patient status and evolving needs.
- Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
- Provide counseling and support to patients and caregivers before, during, and after treatment.
Requirements
- Preferably 2+ years of nursing experience, with a focus on geriatrics.
- Nursing degree from an accredited institution.
