Responsibilities
- Care for your own panel of senior patients in our comprehensive, interdisciplinary model, delivering best-in-class outcomes
- Partner with your Care Center Manager to outperform on the metrics that matter: patient experience, STARS/HEDIS, care coordination, utilization, and cost of care
- If you're mission-driven, growth-minded, and want to help build something better in primary care, we'd love to meet you.
- AI dictation support to cut down documentation time
Requirements
- 3+ years of post-residency experience, including experience managing teams
- Dedicated MA and Clinical Documentation Specialist - your charts come prepped, so you can focus on the patient
Nice to have
- Experience with the Medicare population and at-risk / high-acuity panels
- Familiarity with value-based or other innovative clinical care models
