About the role

This position is responsible for conducting comprehensive assessments of patients' substance use needs and identifying appropriate interventions.

Provides crisis intervention, counseling, and support to patients and family members when indicated.
Collaborates with interdisciplinary hospital staff to promote effective management and treatment of patients with substance use disorders.
Works with treatment team to develop, coordinate, and facilitate placement and discharge plans to ensure appropriate follow up care and continuity of services.
Maintains professional levels of productivity, coordinates, and prioritizes patient care related activities in an efficient and effective manner.
Maintains confidentiality and complies with professional ethics according to professional, departmental and organizational standards.
Performs other duties as assigned

Bachelor's Degree B.A., B.S. or related field Required or
Master's Degree M.A., M.S. or related field Required
4 years Experience in addiction services with a LBSW, LMSW, LMLP, LPC.
LAC, LMAC, LCAC may substitute for this work experience.
Experience in substance use disorder cases, assessments, and referrals to all levels of substance use disorder treatments.
Experience in group work, case work, and community methodologies Preferred
Customer service experience Preferred

Excellent communication and interpersonal skills
Case Note Documentation
Knowledge of organizational strategies and ability to apply those strategies in practice.
Licensed Master's Addiction Counselor
KSBSRB Required or
Licensed Addiction Counselor
Licensed Clinical Addiction Counselor
Licensed Bachelor Social Worker
KSBSRB Plus 4 years work experience in addiction services or related field Required or
Licensed Masters Social Worker
Licnesed Masters Level Psychologist
Licensed Professional Counselor

Abide by Stormont Vail's Remote Worker Policy and will review and acknowledge the Remote Work Agreement annually.

Interviews patients and family members, reviews medical and psychosocial records, and consults with physicians, nursing staff, and other professionals to assess patient needs and conditions.
Refers patients to appropriate internal and external services, including detoxification, medical evaluation, treatment programs, social services, and recovery support resources.
Provide patient education regarding diagnosis and treatment.
Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
