About the role
- This role combines clinical assessment, care coordination, patient and caregiver education, and quality assurance to ensure safe, effective, and person-centered care.
- Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance.
- The Care Manager, RN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely intervention, effective care coordination, and optimal patient outcomes.
Responsibilities
- Develops, implements, manages patient's plan of care.

- The Care Manager, RN is responsible for conducting initial in-home assessments, orientations for new patients and caregivers, and providing ongoing care management and support.
Requirements
- Ability to articulate cases through effective case summaries.
- valid driver's license and car insurance required.
- Proficient in common software and web-based applications (e.g. Case Management Systems, MS Office) with the ability to teach others.
- A passion for providing a high level of customer service, including the ability to show empathy, active listening, and patience.
- Valid and active Registered Nurse (RN) license in the State of Connecicut
- Ability and flexibility to travel (usually by car) within assigned geographic area; valid driver's license and car insurance required.
