Responsibilities
- Collaborates with advanced practice clinicians and/or social worker to develop an action plan
- Able to communicate positively, professionally and effectively with others; provide leadership, teach and collaborate with others.
- Member visits are generally organized by geographic service areas or quadrants to support efficient scheduling and reduce driving time
Requirements
- Required: High School Diploma or GED.
- Ability to travel locally throughout Maricopa County and transport necessary work materials
Nice to have
- Senior care experience.
- Bachelor's Degree
- Bilingual English and Spanish
- Preferred: Bachelor's Degree
- Preferred: Senior care experience.
Skills
- Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.
- Required:
- Solid knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise
- Knowledge of and experience with applying the stages of change or a similar behavior change model
- Able to work independently and remotely.
- Proficient computer skills including Microsoft Office Excel, Word and PowerPoint
- Comfortable working independently in a field-based environment with frequent local travel.
- Ability to navigate changing daily schedules and prioritize member visits across multiple coverage areas.
Compensation
- $44,987.00 - $67,480.00 Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
