- The major functions of the CNS/NP include education, consultation, clinical practice, research, and leadership.
Responsibilities
- Optimize post hospital care coordination including Tele-monitoring and specialist referrals.
- Collaborate with the Enterprise wide teams to improve patient satisfaction and patient care outcomes.
- Perform preliminary/concurrent case reviews to support cardiac quality assurance, quality improvement activities and outcomes management
- Develop or collaborate on patient teaching materials
- The CNS/NP plans, implements, coordinates and evaluates nursing care and clinical programs for patients.
Requirements
- Master's degree in Nursing.
- Must have at least 24 months clinical nursing experience in area of specialty.
- Demonstrated ability to produce results and coordinate projects
- Experience of expertise in quality improvement and evidenced based practice.
- Three (3) years of recent experience in acute nursing and demonstrated clinical competence and in-depth knowledge in the clinical area
- Prior CNS, NP or nursing education experience preferred.
- Excellent verbal and written communication skills, problem solving, conflict resolution skills.
- Computer skills that include word processing and database management
- License/Certification/Registration Requirements
- Current California RN license.
- California certification as a Clinical Nurse Specialist or California licensure as a NP Current
- BLS certification
- Department Specific:
- Stroke Coordinator/Neurovascular
Compensation
- $105.68 - $142.64 USD Hour
