About the role
- Assessment & Planning Conduct comprehensive assessments to evaluate clients' needs, strengths, challenges, and goals.
- Ensure that all services are aligned with the client's goals and best interests.
- Monitoring & Evaluation Regularly assess client progress and evaluate the effectiveness of services received.
Responsibilities
- Develop individualized service plans tailored to each client's specific requirements.
- Connect clients with professional services, community resources, and relevant support programs.
- Advocacy & Support
- Represent clients in discussions with service providers to ensure they receive appropriate care and support.
- Adjust treatment and support plans as needed to improve client outcomes.
- Maintain consistent communication with clients to assess their well-being and address any concerns.
- Build strong relationships with local service providers to streamline client support.
- Provide immediate support and intervention during crises.
- Maintain accurate records of client progress, referrals, home visits, and notable interactions.
Requirements
- Bachelor's degree in a relevant field.
- Minimum of one (1) year of experience in case management or a related field.
- Bachelor's (Required)
Skills
- Bilingual candidates are a plus.
- Strong knowledge of community resources and social services.
- Excellent time management and organizational skills.
- Strong communication and documentation abilities.
Compensation
- $45,000.00 per year
