About the role

  • Assessment & Planning Conduct comprehensive assessments to evaluate clients' needs, strengths, challenges, and goals.
  • Ensure that all services are aligned with the client's goals and best interests.
  • Monitoring & Evaluation Regularly assess client progress and evaluate the effectiveness of services received.

Responsibilities

  • Develop individualized service plans tailored to each client's specific requirements.
  • Connect clients with professional services, community resources, and relevant support programs.
  • Advocacy & Support
  • Represent clients in discussions with service providers to ensure they receive appropriate care and support.
  • Adjust treatment and support plans as needed to improve client outcomes.
  • Maintain consistent communication with clients to assess their well-being and address any concerns.
  • Build strong relationships with local service providers to streamline client support.
  • Provide immediate support and intervention during crises.
  • Maintain accurate records of client progress, referrals, home visits, and notable interactions.

Requirements

  • Bachelor's degree in a relevant field.
  • Minimum of one (1) year of experience in case management or a related field.
  • Bachelor's (Required)

Skills

  • Bilingual candidates are a plus.
  • Strong knowledge of community resources and social services.
  • Excellent time management and organizational skills.
  • Strong communication and documentation abilities.

Compensation

  • $45,000.00 per year
