About the role

- As part of the Medical Home Program, provide outreach, training and technical assistance to pediatric providers in private practice and clinic sites in low-income county.
- Work in collaboration with public health and other child agencies to promote developmental screening and appropriate referrals for services across children and youth populations.

Requirements
- Bachelor's degree in a work-related discipline/field from an accredited college or university.
- License/Certification: None required.
- Ability to apply judgment and make informed decisions.
- Ability to plan, organize, prioritize, work independently and meet deadlines.
- Ability to speak and write effectively at a level appropriate for the job.
Compensation
- Compensation is based on the level and requirements of the role.
- Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data.
- Typically, new team members join at the minimum to mid salary range.
- Minimum to Midpoint Range (Hourly): $37.62 to $48.91
