About the role
- Conduct initial and ongoing spiritual assessments and contribute to individualized plans of care.
- Attend interdisciplinary team meetings and provide insight regarding patient and family spiritual needs.
- Connect patients and families with community clergy, faith leaders, or spiritual resources when requested.
Responsibilities
- Provide spiritual and emotional support to hospice patients and their families in homes, facilities, hospitals, or other care settings.
- Offer prayer, scripture reading, sacraments, rituals, life review, legacy conversations, or other spiritual support as requested by the patient or family.
- Respect and support patients of all faith traditions, spiritual backgrounds, and those with no religious affiliation.
- Collaborate with the interdisciplinary hospice team, including nurses, physicians, social workers, aides, volunteers, and bereavement staff.
- Support families and caregivers through grief, anticipatory loss, crisis, conflict, and bereavement concerns.
- Maintain timely and accurate documentation of visits, assessments, interventions, and care plan updates in accordance with agency policy and regulatory requirements.
- The Hospice Chaplain provides compassionate spiritual, emotional, and pastoral support to hospice patients, families, caregivers, and staff.
- This role honors each person's beliefs, values, culture, and preferences while helping patients and families find comfort, meaning, peace, and support during the end-of-life journey.

- Ability to provide calm, nonjudgmental presence in emotionally difficult situations.
- Required Qualifications
Nice to have
- Bachelor's degree in theology, ministry, pastoral care, counseling, religious studies, or a related field
- master's degree preferred.
- hospice, hospital, or palliative care experience strongly preferred.
Skills
- Compassionate listening and strong interpersonal communication skills.
- Effective documentation, organization, and time management skills.
