Responsibilities
- Collaborates in the collection and assessment of the patient's problem/condition.
- Elicit and document the patient's chief complaint, history and patient needs.
- Collaborates on the development, planning and modification of the Registered Nurse's or Advanced Practice Nurse's nursing care plan.
- Implements aspects of the nursing plan of care as delegated and with demonstrated competencies
- Provides input in the development of policies and procedures to support patient safety.
- Performs the following duties and procedures within the scope of LPN practice: vital signs, phlebotomy, specimen collection, wound care, and urinary catheterization.
- Assists physicians and RNs in other patient care support activities related to referrals, lab work, diagnostic testing, medication ordering, results reporting and scheduling, as delegated.
- Performs other tasks as delegated and within the LPN scope of practice.
Requirements
- Licensed Practical Nurse Licensure by the State of Illinois required.
- Graduation from an accredited school of practical nursing required.

- Documents all observations and interventions in the patient medical record as appropriate.
- Serves as an advocate for the patient by communicating and collaborating with other health service personnel, as delegated.
- Assists in urgent/emergent medical situations, as appropriate, and in accordance with the organization's protocols for medical emergencies and cardiac arrests.
- Cardio-Pulmonary Resuscitation recognition (healthcare provider level) - American Heart Association Accredited.
