About the role

  • Under the supervision of the Program Director the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients.
  • The Care Coordinator will coordinate individualized service including orientation, transportation, doctor visits, and schedule.
  • Duties and Responsibilities: Ensures programs and services are implemented at the highest standards and clients receive the highest level of care.

Responsibilities

  • Document shift reports and share pertinent information with the team.
  • Coordinate the transportation schedule for all clients and drive clients as needed.
  • Document all transportation and group activity in EMR.
  • Maintain the confidential nature of clients and related activities.

Requirements

  • Must have a valid driver's license and eligibility for insurance with agency's vehicles.
  • Must have valid First-Aid & CPR Certification or be willing to obtain.
  • Minimum of two (2) years driving experience required.
  • Must have strong written communication skills.
  • All documentation in EMR must be submitted within 24 hours.

Nice to have

  • The Care Coordinator will have a minimum of a high school diploma, Bachelor's degree preferred.
  • Minimum of 1-year working with kids in a therapeutic setting Experience with junior high or high school students is preferred.

  • Engage in appropriate written and verbal communication with families and other professionals.
