- The Outpatient portion of the program is centralized through our outpatient clinic within Saint Francis Hospital.
- This clinic specializes in various degrees of heart failure with a focus on improving our patients' quality of life and optimizing and improving their disease process.

- Certification: ACLS and Stroke Certification preferred.

- This RN provides heart failure disease management education and assists in managing our patients within the Outpatient Heart Failure Program.
- You will assist with in-basket inquiries within the electronic health record and necessary phone calls to patients and other providers within the ministry to assist in managing patients care.
- You will provide inpatients that are late day discharges with scheduled appointments and bedside education prior to their discharge from the hospital.
