About the role
- We're looking for a Registered Nurse who brings strong clinical judgment, calm communication, and a genuine passion for caring for their patients.
- This is a fully remote position with consistent weekday hours (Monday-Friday, ET).

Responsibilities
- Maintain high-quality documentation in the clinical platform: assessments, interventions, outreach attempts, triage decisions, and follow-ups, with strong accuracy and follow-through.
- You'll work alongside our Nurse Practitioners, physician leaders, and care team to deliver a concierge-level experience that helps patients stay healthy, independent, and ahead of problems.
Requirements
- You communicate with warmth and clarity across phone, video, and chat, and you can adapt your style to the person in front of you.
- Tech savvy and fast-learning across tools like EMRs, chat platforms, video calls, dashboards, and spreadsheets
- Good judgment and professionalism: knows when to loop in NPs and physician leaders, protects privacy, and communicates accurately under time pressure

Nice to have
- Experience with RPM or CCM programs or value-based care
- Spanish or other community language skills
- Prior startup or high-growth environment experience

- 3+ years of nursing experience (care management, primary care, geriatrics, ambulatory, or telehealth a plus)

Compensation
- $58,000-$72,000/year, based on experience
