About the role
- Pharmaceutical care will focus on patients enrolled in primary care, palliative care, advanced illness care, and hospice care programs and will be primarily focused on caring for geriatric patients.
- This pharmacist will play a key role in the patient-centered medical home, incorporating principles of population-based medicine and providing comprehensive medication management under established collaborative practice agreements.
- This position will coordinate the expansion of pharmaceutical care services within Housecall Providers primary care and palliative care services.

Skills
- Ability to critically evaluate clinical pharmaceutical and medical literature and apply principles of evidence-based medicine
- Must be highly motivated and have the ability to work independently

- Excellent written and verbal communication skills
- Excellent customer service skills
- Ability to work in diverse settings, such as residential care homes, private homes, and other areas of the community
- Ability to manage multiple tasks

- Ability to work effectively with diverse individuals and groups
- Ability to learn, focus, understand, and evaluate information and determine appropriate actions
- Ability to see, read, hear, and speak clearly for at least 6 hours/day
Compensation
- Bonus - SIP Target, 5% Annual
