About the role
- This role will complement our interdisciplinary teams by addressing the psychosocial needs of our patients.
- Despite a mountain of evidence, less than 1 in 10 people with congestive heart failure are prescribed guideline-based medications proven to save lives and prevent hospitalization.
- Only 3 in 10 people with diabetes have ever received comprehensive diabetes education, and in the lucky few who have received education, it is rarely integrated with evidence-based pharmacotherapy.
Responsibilities
- Establish trust and build strong relationships with our patients
- Emotional and caregiver support
- Navigate family/caregiver support
- Work with family members/caregivers to help them understand the patient's needs and how to support them

Skills
- Must Haves
- Master's degree in social work and active license
- Minimum of 2 years expertise in the medical field
- Comfortable with technology and a fast-paced environment
- Nice to Haves
- Bilingual in English and Creole, Spanish, or Vietnamese.
- Experience in health insurance (Medicare Advantage a plus)
- Experience in geriatrics
- Self-starter with ability to succeed in a work-from-home environment
- Strong interpersonal and communication skills
- Comfortable working with patients over video and phone
- You love helping others improve their health and navigate a complex healthcare system with compassion, empathy, and warmth
Compensation
- $70,000 - $80,000 / year
- The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting.
