Responsibilities

  • Develop and execute personalized, patient-centered care plans that address the whole person
  • Deliver monthly Chronic Care Management (CCM) visits and oversee CCM documentation
  • Collaborate with senior living leadership and care teams to optimize care plan execution and patient outcomes
  • Deliver thorough, unhurried care with the time your patients need
  • Build genuine connections by meeting patients where they live

Requirements

Nice to have

  • NP certification with a genuine passion for geriatric medicine (prior geriatric experience preferred but not required)

Compensation

  • The base salary range for this role is $135,000 to $160,000 plus generous bonus, equity, and benefits.
