Responsibilities
- Perform face-to-face intake for consumers who have been referred to MHC and assess for care management, medication management or therapy eligibility.
- Encourage consumers to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships.
- Develop and implement treatment plans based on clinical experience and knowledge.
- Collaborate with other staff members, primary care practice partners including pediatricians, doctors, nurse practitioners, office staff, and referral sources as assigned to perform clinical assessments and develop treatment plans.
